Application Process

Applying to Copper Ridge

On behalf of the prospective resident, a family member or responsible party will submit an application for admission to one of Copper Ridge’s programs. Once eligibility is determined, our admissions coordinator will arrange a preadmission meeting with the family member or primary caregiver (and the prospective resident as appropriate). During this meeting, the admissions coordinator will explain the details of our program, review financial information and discuss the prospective resident’s history, personality, psychosocial and medical needs.

Evaluating Needs

To help us develop a care plan tailored to each prospective resident’s needs, we offer a thorough assessment. This assessment enables our physicians and social workers to determine the prospective resident’s functioning level and cognitive abilities.

Planning Care

Our social worker, a registered nurse and other healthcare professionals will meet with the family to review the care plan for the new resident and to further discuss our commitment to their family member.

Easing the Transition

Family members may wish to join the new resident for a meal in our dining area or spend some private time together to help him or her become accustomed to the new environment.

Staying Involved

We encourage family and friends to remain involved through volunteer programs, structured activity times, our open visiting policy and opportunities to share occasional meals. Our educational and support groups are available to our residents’ families and friends and to the public.
